ADDITIONAL RESOURCES

Below are additional resources Crush Cancer has put at your disposal to help you get ready for the event!

For All Participants

All Crush Cancer participants are welcome to use the documents below to help us spread the word about the cause and to prepare for the event. Please note that each person who plans to ride a bicycle on Sunday, May 5 will need to fill out a waiver form.

Print this form and carry it around with you, wherever you go, so you can give it to your friends, family and colleagues. It's a great way to tell them about Cycle for Survival and invite them to join the battle on May 5, 2019.

If you prefer email, feel free to use this electronic flyer and send it to your friends, family and colleagues. It's another great way to tell them about Cycle for Survival and invite them to join the battle on May 5, 2019.

Feel free to print this form, read it and sign it ahead of time. If you forget to bring it with you on the day of the event, we will have extra copies available on site.

NOTE: Participants must be 14 years of age or older to ride a bicycle at the event.

For Team Captains

Below are several forms you can download to help you spread the word about Cycle for Survival and encourage people to join your team! There is also a check list that will walk you through important steps to follow in order to be a successful team captain.

If you prefer email, feel free to download this form and complete the missing information before sending it to your friends, family and colleagues electronically. This is another great way

to encourage them to join your team!

Follow up with your friends, family and colleagues via email and ask them (again) to join your team! This email template will serve as a reminder to them and it provides detailed instructions on HOW they can join your team.
